This notice explains how Crestpath Health handles what you send through the price check request on this site.

What we collect

Your business or practice name, chosen inquiry, current purchases and pricing, any business invoice you upload, and your name and contact details so we can follow up. No account is created by this inquiry.

How we use it

We use your business and contact information to review your request, verify eligibility and follow up to help you see how your current pricing compares. We may share your information with a pharmacy or research lab partner for your selected inquiry so they can prepare a comparison and contact you about pricing, service and next steps. This inquiry may become a lead for that partner. Pharmacy and laboratory inquiries are kept separate. By submitting the form, you agree to this use and partner sharing. There is no obligation to switch and no guarantee of savings.

Crestpath Brief subscriptions

If you choose to subscribe, we store your email address, consent wording and version, request source and dates, confirmation status and unsubscribe status separately from quote inquiries. Your checkbox consent subscribes you directly; we do not send an automatic confirmation email. We use this information for research and pharmacy-policy updates, not partner leads. Quote requests never subscribe you automatically. Use the private unsubscribe link shown after signup or in a future brief to stop updates. We retain unsubscribe status to prevent further sending. We use our database and email providers to run this service. Hashed network identifiers are kept briefly to limit signup abuse.

Website measurement

We use first-party, short-lived session identifiers and session cookies to count page visits, inquiry steps, validation errors and saved requests. We also count brief subscription attempts and completions separately. We record only step names, page, inquiry type and broad traffic source, not your form answers, contact details, product or price text, patient information, full referring URLs or advertising click IDs. We do not use session replay or third-party analytics for this measurement. Raw measurement events are retained for up to 30 days and cleaned as new events arrive. Browsers sending Do Not Track do not create measurement events. You can also block cookies or scripts in your browser.

Patient information

This site is for business inquiries only. Please do not include patient information in your request or in any file you upload.

Questions or removal

To ask us to update or delete what you submitted, reply to any follow-up message from Crestpath Health and we will handle it.
